Education and Training in the Health Sector, UWI/RHA Teaching Hospital Relationships

Dean, Faculty of Medical Sciences

 

  • Medical teaching started in 1967 at Port of Spain General Hospital (POSGH)

 

  • 4th year teaching was started in 1976

 

  • Teaching of Obstetrics & Gynecology was transferred to the Mt. Hope Women’s Hospital in 1981

 

  • IADB Report of 1971 recommended the development of a second University Medical Centre in the Eastern Caribbean

 

  • Faculty Steering Committee was set up

 

  • Eric Williams Medical Sciences Complex (EWMSC) was opened in 1986 by Prime Minister George Chambers

 

 

 

UWI/Government agreement included the following:

 

1.             The Complex shall provide service as well as undertake teaching and research. The Hospitals will be teaching Hospitals.

 

2.             The Authority shall assume responsibility to operate, construct, equip, furnish, maintain, secure and repair all property

 

3.             Teaching and research in Veterinary and Dental Services

 

4.             The University would be expected to assume full responsibility for supplies and materials required for research, teaching and administration of its activities

 

5.             Arrangements should be made for University personnel to hold contracts with the Authority to provide clinical services in the hospitals.

 

6.             University contracts shall include a requirement that the appointees provide service in the hospitals on the terms and conditions agreed to between the University and the Authority.

 

§               Phased transfer of medical services from Port of Spain General Hospital to EWMSC

 

§               The 1995 Regional Health Authority Act incorporated the intent of the relationship.

 

§               In 1989 - 92 students were enrolled

 

§               In 2001 - 212 students

 

§               The Faculty is a self-financing Faculty whose income depends on a Government subvention and student fees.

 

§               Any change in our financial responsibilities to the Regional Health Authority would require adjustments to the subvention or the fees.

 

§               The Educational Philosophy is based on the use of the PBL methodology and some didactic reaching

 

§               This is the preferred method of teaching worldwide.

 

§               The curriculum is integrated both vertically and horizontally

 

 

 

Partial commissioning of the Hospital has affected the Faculty’s ability to realize these education goals

 

§               This particularly affects the clinical teaching from Years 3-5

 

§               Accredited Internships Posts are now available at Scarborough Regional Hospital, Sangre Grande, San Fernando, EWMSC and Port of Spain General Hospital

 

Dental School

 

§               DDS Degree has suffered from negative publicity

 

§               Four staff members are graduates who now hold specialist qualifications

 

§               University staff provides most of the Clinical Services at the Dental Hospital

 

§               Planned student number of 25/year will now necessitate the utilization of the full Dental Hospital

 

§               The current Curriculum means that up to 125 students and interns will be attached to the hospital

 

§               Non-Functioning Dental Operating Theatre

 

§               Need to provide accommodation for the Dental Students and interns in the public health sector to provide Dental Care at Health Centre and at Accredited Hospitals

 

§               The Dental School needs to occupy the full Dental Facility for teaching and thus the University should take over the running of the Dental Hospital

 

§               Dr E Scipio has recently been appointed the Clinical Director of the Dental Hospital and this move will allow the Dental School to make a significant contribution to the management of the Hospital

 

 

 

Veterinary School 

 

§               The only school in the Caribbean which provides a full 5-year training programme

 

§               The degree already has advanced standing in the USA

 

§               The Hospital is not a major priority of the RHA and thus it is poorly maintained

 

§               The University should take over the running of the Hospital

 

§               Professor Ezeokoli is the Clinical Director of the Hospital

 

 

 

Pharmacy Programme

 

§               B.Sc. (Pharmacy) is recognised in Canada

 

§               All regional pharmacists will be required to hold a B.Sc. (Pharmacy) Degree

 

§               Recognition of the Role of Clinical Pharmacists is required

 

 

 

Nursing Degree

 

§               B.Sc. (N) is approved by the relevant Committees at St. Augustine and B.U.S. has been asked to ratify

 

§               Need to finalize the funding and Staffing to allow the introduction in the Academic Year 2003/2004

 

 

 

Other Programmes

 

§               DM – Anaesthetics, Obstetrics & Gynaecology, Psychiatry, Radiology and Orthopaedics

 

§               DM (Child Health) and DM (Surgery) in the next year

 

§               In the Future:

 

¨                   DM (Medicine), Emergency Medicine, and M.Sc. in Speech Disabilities, Family Medicine, Public Health and Occupational Health

 

¨                   Diplomas in Family Medicine already exists and Diplomas in Occupational Health, Sports Medicine and Sports Psychology are planned

 

Items for Consideration

 

§               Faculty of Medical Sciences will collaborate with M.O.H, R.H.A’s, Medical, Dental, and Veterinary and Pharmacy Regulatory Bodies

 

§               Need to ensure adequate Clinical Material at EWMSC as well as other Teaching Hospitals

 

 

Agreements and Protocols defining the relationship between UWI / NWRHA / EWMSC need to be negotiated.

 

§               They should include:  

¨             Contracts for Clinical and Paraclinical UWI Staff for service at the Regional Health Authority Hospitals and Health Centres 

¨             Agreement of access of University Clinical Staff and students to patient material

 

§               The provision of adequate facilities for interns, including on call rooms, structured educational programmes and adequate time off to allow for continuing study. 

¨             Greater use of the Paraclinical Staff to supervise the laboratory service  

¨             Define the 2 parties responsibilities for Security, Air Conditioning, Electricity, Telephones and other infrastructural facilities 

¨             Provision of Training Posts in the Major Specialties 

¨             Research Activity and Expansion of the Physical Plant

§               Impact of the Faculty on the Development of the Delivery of Health Care 

§               We stand willing to assist in reaching the goal of “Health for All” 

§               I feel that it is important that this investment by the people of Trinidad and Tobago achieves its full potential.
